Job Details Job Summary:

The Consult Clinician LCSW or LMSW-CC has experience in the treatment of mental health, addiction, or co-occurring disorders, and forensics. The Consultation Services Team provides psychiatric/chemical dependency triage and assessments at varying levels of urgency for all age groups on a 24 hour basis. The work environment includes telemedicine on site at The Acadia Hospital, interview rooms at Acadia Hospital, the Psychiatric Observation Unit, inpatient units at Acadia Hospital and outpatient areas at Acadia Hospital. Work hours vary. The clinician also provides assessments and evaluations for minor to critically ill clients on a scheduled basis. The clinician works under the direction and supervision of the Clinical Supervisor for Consult Services and the clinical supervision of an Acadia Psychiatrist or Psychiatric Nurse Practitioner. Acting as a liaison with many community services and providers, the clinician is responsible for negotiating/advocating for the most appropriate, least restrictive services for the client. The Consultation Services Clinician models a multi-disciplinary approach to providing quality patient care as defined in the Acadia Hospital/Acadia Healthcare policy and procedure manual. This person performs duties and/or responsibilities as assigned by the Manager or Supervisor for which s/he is qualified.

Job Responsibilities:

· Completes a comprehensive, recovery-oriented bio-psycho-social evaluation (including five axis diagnoses, mental status exam, and case formulation) by collecting data which enables the social worker to identify potential therapeutic targets for active treatment in collaboration with the client within four hours of the initial contact.

· Develops initial master treatment plan and updates treatment plans in collaboration with client and treatment team as treatment needs change, or within established time frames, to identify relevant recovery oriented goals and objectives for active treatment.

· The master treatment plan is updated according to regulatory regulations.

· The master treatment plan identifies effective interventions to achieve outcomes, patient's motivation, health/spiritual beliefs and functional capabilities and strengths.

· The master treatment plan reflects a continuum of services to be provided.

· The master treatment plan reflects the clinicians' ability to analyze, classify, and interpret social and personal data, including trauma assessments and initial evaluation of risk to self and others.

· Regularly considers and involves ancillary services within active treatment.

· Provides individual and group clinical supervision to other staff as assigned, ie. nursing staff, support and ancillary staff.

· Mentors all new staff to ensure high quality services is provided.

· Provides clinical supervision to conditionally licensed providers as requested by clinical leadership.

· Implements interventions that are consistent with service plan and developmental level and needs of client to elicit therapeutic change.

· Interventions are modified based on continued assessment of the patient's response to treatment.

· Interventions are performed according to the clinician's level of education and practice.

· In the absence of the clinical associate, provides case management to coordinate comprehensive health services (including primary care and dentistry), appropriate referrals to community providers and support systems to ensure continuity of care following discharge.

· Completes documentation that demonstrates active client involvement in treatment.

· Completes documentation that reflects barriers to treatment progress.

· Expected treatment outcomes are patient oriented, evidence based, therapeutically sound, realistic, attainable and measurable.

· Demonstrates reflective/active listening, feedback, summarizing, reframing, empathy, support, problem solving, coaching, and psycho-education skills.

· Consistently complies with the policies and procedures as set forth in the Clinical Policy and Procedure Manual.

· Maintains and abides by ethical standards set forth by the facility and Maine State Licensing Boards.

· Maintains requirements of licensure.

· Fosters the development of teamwork within department as well as across departments and with external providers.

· Serves as a role model by developing respect, confidence, cooperation and participation in the work environment.

· Maintains all requirements for credentialing.

· Reviews all meeting minutes.

· Gives and receives critical feedback on effectiveness of clinical interventions through regular peer review, scheduled staff meetings and supervisions.

· Follows through with commitments to the patients and other providers in a timely and professional manner.

· Communicates plans and coordinates with other team members the scheduling of meetings with patients.

· Utilizes time effectively and can critically evaluate time management relevant to the effective operation of respective department.

· Considers feedback from all disciplines when evaluating, planning and implementing interventions.

· Provides feedback to all disciplines within ones scope of practice when evaluating, planning and implementing interventions.

· Ensures and supports a therapeutic milieu through patient and family advocacy.

· Seeks appropriate supervision from responsible MD (s).

· Completes the written/dictated assessment within four hours of the initial contact.

· Completes the Suicide and Violence Risk Assessments within four hours of the initial contact.

· Ensure that assessment is complete and clearly specifies the level of acuity for each case.

· Completes all necessary paperwork and billing and delivers it to the appropriate place prior to the end of assigned shift.

· Completes all database entries prior to the end of assigned shift.

· Notifies database keeper of any updates to the database prior to the end of assigned shift.

· Identifies and states the present admission criteria as specified by the hospital admission policy should the patient require admission to the hospital.

· Ensures all cases are reviewed with covering Psychiatrist/LIP prior to disposition.

· Negotiates/advocates for the most appropriate, least restrictive service for the client.

· Follows through with commitments to the patients and other providers in a timely and professional manner.

· Utilizes timely effective and can critically manage time to efficiently ensure that all priorities are covered.

· Participates actively in group planning, implementation, and processing at the departmental level as well as hospital wide as requested.

· Provides direct service as need in all hospital hotspots.

· Continually fosters a work environment that supports strong therapeutic alliance and relevant treatment structures to eliminate use of restraints.

· Possess the core competencies of self-management, reflective practice, collaboration, systems thinking, initiative, and flexibility.

· Works cooperatively with other team members towards the maintenance and improvement of the therapeutic milieu via education/assistance to other team members in understanding the patient/family problem from the clinical perspective.

· Gives and receives critical feedback on effectiveness of clinical interventions through participation in staff and treatment team meetings.

· Supervises students in their assessment, intervention and systems abilities while helping established a high level of clinical skill.

· Possess a working knowledge of the medical and surgical component of the psychiatric evaluation.

· Assist in maintaining a work environment free from recognized hazards that create a risk of injury to employees, patients or visitors and that all accidents and incidents are reported by employees and properly investigated. When appropriate, assist with the return of all workers with work related injuries and illnesses to gainful employment.

Job Requirements:

· Qualified individual must be a LMSW-CC.

· Ability to be flexible and exercise independent judgment.

· Demonstrates collegial and multi-disciplinary relationships along the treatment continuum.

· Experience in hospital-based clinical treatment services preferred.

· Completes Healthstream Mandatory Education on a yearly basis.

· Attends all mandatory safety related education programs.

· Follows safety procedures to prevent injury to self and others.

· Carries assigned keys and wears hospital ID badge at all times.

· Refrains from the abuse of chemicals.

· Maintains patient confidentiality at all times.

· Represents the organization in a positive and professional manner in the community.

· Maintains regulatory requirements, including Joint Commission, Federal and State.

· Complies with all organizational policies regarding EMHS's Code of Conduct and ethical business practices.

Physical Demands:

· Visual acuity, color vision, hearing acuity, and tactile sensation are necessary to respond to patient response and requests.

· The ability to read and speak English, to communicate and document information.

· Must possess the manual dexterity and a degree of body strength and flexibility to respond to aggressive patient behavior.

· The ability to reach and stretch intermittently.

· Time may be spent sitting at a computer, collaborating with other healthcare team members.

· Standing and walking comprises some of the workday.
