Job Details GENERAL DESCRIPTION/STATEMENT:

The Diabetes Nurse Educator is responsible for the oversight and implementation of the complete Diabetes Self-Management Program, including patient encounters, class instruction, procedures and protocols, client records, statistical reports, charging, marketing and recertification. He/she must also respond to emergent diabetes-related needs outside of the DSMTP. The Diabetes Nurse Educator coordinates and/or assist with diabetes continuing education for nurses across the organization.

ESSENTIAL DUTIES:


1. Coordinate and implement all aspects of the Diabetes Self-Management Training Program, including biannual recertification efforts, according to state and federal guidelines.

2. Document and implement patient plans of care.

3. Provide input to medical and nursing staff relating to diabetes care and management.

4. Respond to emergent patient needs throughout the hospital and outpatient practices.

5. Coordinate and/or assist with assigned educational offerings.

6. Accept responsibility for professional growth and development.

7. Participate in at least 75% of Educational Services staff meetings.

8. Serve on Policy and Procedure Committee and other ad hoc committees as assigned.

9. Comply with all JCAHO, state and federal requirements and regulations.

10. Demonstrate a commitment to the philosophy and objectives of the TAMC organization.


OTHER RESPONSIBILITIES:


1. Performs other work related duties as assigned.


LICENSE REQUIREMENTS:

1. Current Maine Registered License or license eligible.

2. Current Diabetes Educator Certification (completed or in process).


AVAILABILITY REQUIREMENTS:

Variable work schedule requires business hours with some evening work.


EDUCATION SKILLS, EXPERIENCE REQUIREMENTS:


Requirements: Must haveState of Maine Registered Nurse licensure (or license eligible), and be willing to work toward certification as a Certified Diabetes Educator (CDE).
